New Orthodontic Patients: We look forward to meeting you to discuss possible orthodontics for your child. Please print and complete a copy of our Orthodontic Health History form and bring it with you to your first appointment. You can also mail or FAX the form to us. Appointment Times: Our office staff will work with you to find convenient appointment times for you and your child. We do our best to keep school time appointments to a minimum. However, there are certain procedures that are performed in the morning, or early afternoon, that may require your child to be seen during the school day. Rescheduling or Canceling Appointments: We require a minimum of 24 hours notice if you need to cancel or reschedule an appointment. A charge of $25 (per child) will be assessed for same day cancellations or missed appointments without prior notification. If you need to reschedule or cancel your child's appointment, please call our office as soon as possible so that we may give that time to another patient. Staying on Schedule: We recognize that your time is valuable and we try very hard to stay "on schedule" every day. We understand that unexpected things happen that may prevent you from arriving on time. There may also be emergencies or unexpected patient conditions that cause us to run slightly behind schedule. As a courtesy to you, if this occurs, you will be informed so you can decide whether you want to wait or reschedule your appointment. If you are running late, we will decide when you arrive if there is enough time to complete your treatment in the allotted time. We do this out of courtesy for the patients scheduled after you. Dental Insurance: We are happy to file claims for our patients with dental insurance. Please bring your current insurance card with you to each appointment. Once your insurance company has reimbursed our office, we will send you a statement detailing any fees that your insurance did not cover. We ask that this balance be paid within 30 days. Orthodontic Dental Insurance: We will assist you in filing your orthodontic insurance claim to ensure you receive your maximum insurance benefit. However, we do require all payments for orthodontic services be paid directly to our office by the patient. Most orthodontic patients set up a monthly, no interest, payment plan with our office to cover the orthodontic fee. Your insurance company will then pay you directly throughout the course of treatment. Payment schedules and policies vary among insurance companies. For more information on DENTAL INSURANCE, click here. Payment Policies: For our patients who do not have insurance, payment is requested at each appointment. We accept cash, checks, MasterCard, VISA, Discover & American Express. If considerable dental work is required, alternative payment plans are available. Please contact our office before your appointment if you have any questions. Medicaid Policies: If you receive Medicaid, you must bring a current card to each appointment . We reserve specific appointment times to see our Medicaid patients each day. |
